Fix uppercase item attribute modifier names (#3780)

* Check for hide attributes flag, and "Name" -> "AttributeName"

* Operation tag is not required?

* Only process each modifier once

* Ignore `minecraft:` namespace if present

* No `Operation` is implicitly ADD, fix knockback_resistance check
